This causes the blood vessels to constrict, which decreased blood flow to the muscles and causes more pain than normal. There are typically three factors that can cause APS: injury, illness, or stress. People with APS have increased pain with stimulations that might not be painful for people who do not have APS, such as light touch or temperature changes. However, winter weather does not have to prevent you from staying consistent with your exercise plan. Knowing some key tips and techniques can help you beat the wintertime woes and stay safe while being outside. Staying Warm First, be sure to dress in warm but thin layers. A base layer of light synthetic material will help remove moisture from your skin keeping you warm. Use a breathable outer layer such as a rain jacket that will block the wind and keep the cold out. In extremely cold conditions a thermal middle layer should also be used for extra warmth. Avoid cotton as this will hold moisture in and keep you cold. Heat will escape any exposed area so be sure to keep your hands, ankles, and head covered. I like the Nike Thermal running gloves and hat as these seem to provide the best all-around comfort for most outings. For extreme conditions, I wear my Nike gloves under thicker wool gloves. To keep your feet warm in extreme conditions you can wear thinner running socks, under thicker wool socks. My favorite running socks are Feetures as they keep my feet dry and provide a snug no-slip fit. Wear shoes with minimal mesh if possible to reduce sloshing around inside your shoes in the wet conditions. Most trail running shoes should provide some moisture resistance. I use the Altra TIMP Trail which is slightly water resistant and has drains to help keep your foot dry. Also, in extreme conditions, you should wear a scarf or ski mask to help reduce the irritation to your lungs and bronchial tubes. Breathing in primarily through your nose, and exhaling through your mouth will help to warm the air and reduce the risk of coughing or asthma-like symptoms that could arise. Breathing in through your nose decreases the total amount of oxygen you can take in, so more than likely you will need to run at a slightly lower intensity or pace to maintain this style of breathing. Your body temperature will rise during the course of a walk or run so to avoid overdressing plan on starting the run feeling "slightly chilly". This will prevent overheating and having to carry multiple items of clothing to finish the run. Another tip is to start your run into the wind, as this will be the most difficult part, allowing you to finish with the wind at your back. Visibility As the daylight hours of the day shorten many of us find it hard to schedule a run when the sun is out. To avoid potential injury be sure to dress in highly reflective gear from head to toe. Wear a headlamp to help light your way to avoid any missteps. Reflective vest, reflective strips, and blinking lights can help you stand out and be noticed. Poor footing Running through snow and ice can prove to be challenging and pose certain risks of injury. Besides slipping and falling you are at increased risk of overuse injuries to the Achilles tendon and other body parts. Plan your route to be as clear of snow as possible, and with minimal traffic. You may have to decrease your typical stride length and increase your cadence. This allows your legs to land closer to your center of mass, and give you an increased ability to correct yourself if you hit a slick spot. Avoid running on the ice at all causes. It is nearly impossible to avoid injury when running on ice. If it is icy, find a treadmill or run on an indoor track. These can pose potential risks in themselves so be sure that you are comfortable using a treadmill* Other Tips Maintaining hydration when training in the winter months is important because your body can still become dehydrated in cold weather. Be sure to hydrate throughout the day, and if you will be running over 30 minutes have a hydration source with continued hydration every 15-30 minutes thereafter. To avoid muscle strain, especially if you will be running at high intensities, a proper warm-up of mobility drills and light stretching is crucial. This can be performed inside one's home or gym to help increase your body temperature. Wear sunscreen and chapstick with SPF to protect your skin from damage. Wear polarized sunglasses to protect your eyes from the sun and the bright glare off the snow. I have recently been using a pair of Goodr glasses that I really enjoy. Posture is something that seems to go by the wayside as we get older or even just as we lead sedentary lifestyles. This can also happen in younger individuals who spend lots of time sitting over textbooks/homework and computers/cellphones. We spend most of our lives sitting forward, soon even upon standing and walking this posture begins to feel normal. Many times, it even feels comfortable to let those postural muscles chill out and it isn’t until you see your reflection in the mirror or a picture from an event that you start to think “when did I become so hunched over?”. But there are other reasons that it does matter, the health of your shoulders and neck. Posture itself is a habit, and this habit can be changed with a lot of self-awareness and constant diligence. Some situations can occur with aging or there are spinal conditions where the upper back is structurally rounded, and we call this Kyphosis. It is not to say that Kyphosis makes it impossible to regain some function or motion in the spine, but full correction of this posture can be very difficult. There is always a good reason to try to correct your posture, not only will it make you feel more confident and attractive, but you can avoid or even fix issues with your neck and shoulders. While it can be comfortable to sit with bad posture for short periods more people will find that with sustaining this posture over long periods that the neck, upper to mid or low back begins to hurt. Why does bad posture hurt my neck? The rounded posture that I am speaking of puts the cervical spine in a precarious position that is abnormal for the natural curve of the spine. From the cervical, thoracic, lumbar and sacral spine there is a natural curve that all parts are meant to follow. These natural curves of the spine are the optimal positions for the trunk in relation to the connecting bony and muscular structures. In these proper positions, the body is most capable of performing activities without injury or stress to the spine itself or the connecting areas such as the shoulders. How does bad posture hurt my shoulders? The relationship between the neck and shoulders is closely tied as the muscles that run through the back assist with motions through the shoulders and the upper trunk. Additionally, on the back, there are bony structures called the scapula or shoulder blades. The shoulder blade moves in relation to the humerus such as when lifting your arm overhead. If the scapula does not move in perfect conjunction with the ball in socket movement occurring at the head of the humerus it can disrupt the ability to move the arm in most directions. For example, if you lift your arm straight ahead and motion occurred from the glenohumeral (shoulder) joint alone and without assistance from the scapula, you would only be able to raise your arm 60 degrees. To put that in perspective 60 degrees is only a third of the range you should have, as in order to have your arm all the way up and straight with your head is 180 degrees. Another thing to consider when the shoulders are rounded forward is the reduction of space and unnecessary pressure put on the many tendons running around and under the bony structures of the shoulder girdle, and this is a large contributor to the dreaded condition coined “impingement”. Okay, so how does physical therapy help with that? Your physical therapist will assign pectoral stretches and scapular strengthening exercises as well as activities that help to reintegrate good posture into daily activities. As bad posture is sustained day in and day out the pec and chest muscles become tight and the scapular muscles become loose and week. Hence the reasoning that the shoulders become rounded and slumped and the shoulder blades rest far apart or even “wing” outward. Activities within physical therapy will help you to re-establish what it feels like to be in good postural positioning via shoulder blade squeezes and activities such as laying over a foam roll which will stretch open the chest and pull the shoulders back. Especially at the point, that shoulder impingement is occurring, it would be beneficial to strengthen the muscles of the rotator cuff. As the rotator cuff muscles have a long all-day everyday job, these activities will be best as high reps and low weight. Additionally, to help re-establish good arthrokinematics that will avoid “pinching” or irritation at the shoulder, your Physical therapist will perform shoulder and shoulder blade mobilizations to help establish a good rhythm between these essential structures. Well, maybe not. Many people with arthritis experience increased joint pain during the cold winter months. The best way to combat this increase in arthritic pain is to stay active! While your joints might be achy and might have a bit of increased pain with activity initially but stick with it and the activity will benefit you in the long run! Here are five great ways to stay active with arthritis in the winter months. 1. Bundle up and walk outside. If the weather is decent, take a walk outside like you would in the warmer months, just wear warm clothing. Make sure to wear layers so that you can shed them if you get too warm. Waiting until the afternoon is ideal so that the weather is at its warmest. 2. Walk around an indoor mall. If the weather outside is too chilly, a great way to get in some good activity is to walk around an indoor mall. The space of the mall gives you plenty of room to stretch and walk around just as you would outside. 3. If you have a gym membership, utilize it. If you don’t want to go to the mall but have a gym membership, go walk or bike at the gym instead. If your gym has a pool, even better! Swimming is a great to exercise for people with arthritis, especially if the pool is heated, because it unloads the weight from the joints. Stretching and gentle strengthening exercises are also very beneficial for alleviating arthritic pain. If you are unsure of how to use equipment in the gym correctly, have a trainer show you how to use the equipment to avoid injury. 4. Exercise in your house. If the weather is too icy or if you don’t have access to an indoor mall or gym, exercise in your home. There are several exercises that are great to perform at home. If you can tolerate stairs, walk up and down your stairs. Walking may be difficult if you don’t have a treadmill, but there you can stretch and perform bodyweight strengthening exercises very easily with everyday household equipment. 5. Take an exercise class. Many gyms and studios offer yoga, Pilates, tai chi, and dance classes. Look up some studios in your area and sign up to take a class or two. Be careful with some of these classes, though, and make sure that they will work for your fitness level. Check with the instructor before you sign up and ask how difficult the class will be to determine if it is right for you. Don’t let the cold weather keep you from moving! Being stationary will likely increase your arthritic joint pain more than what the cold weather will. This is because ACL tears are typically season-ending, especially with the most current research advocating for longer recovery times as it has been shown that the ACL repair may not be fully healed for 1-2 years after surgery. But before we get too far into recovery and prevention, we need to answer the question, "What is an ACL tear and why is the ACL so important?" ACL stands for Anterior Cruciate Ligament and is one of the main stabilizing ligaments in the knee. It prevents the tibia (shin bone) from coming too far forward from the femur (thigh bone), keeping the knee stable and allowing you to run, cut, and jump without your knee "giving out." So, how do we prevent ACL tears? The best way to prevent an ACL tear is to increase and maintain good strength throughout your core and legs, especially in the hips. Along with maintaining good strength, it is extremely important to have good jumping and landing mechanics. Many times, improper landing technique puts the knee in a very vulnerable position, increasing your risk for a tear. This is especially prominent in teenage girls. So learning and practicing proper jumping and landing will help decrease your risk. Another great way to decrease your risk of an ACL tear is to play multiple sports a year and not specialize in one sport before the age of 18. Playing multiple sports allows your body to learn to move and react to different stimuli and therefore your body will become stronger and more equipped to handle the demands of different sports, as well as preventing overuse injuries, which furthers your risk for ACL tears. However, even the best and strongest athletes will get ACL tears despite how well they train. Sometimes, these injuries are inevitable, especially with contact sports where movements can be unpredictable. So, if you do end up suffering from an ACL tear, know that recovery to the point where you are as good (or better) than before is very possible with the right physical therapy. Genetics, lifestyle, hobbies, and past injuries play into each person’s state of health. In some cases, with high severity or unresponsive to conservative treatments, the initial recommendation may be surgical consult. A surgical consultation can even reveal options other than surgery such as injections, braces, assistive devices, or epidural. In situations where other treatments have not been attempted, the injury is acute, or diagnosis is unclear many primary physicians will suggest trying physical therapy. X-rays may be performed in the case that fracture, arthritis, or possible break could be suspected. Many times, Magnetic resonance imaging is not the first imaging to be performed and may even be required by health insurance to be performed only after physical therapy or other conservative treatment has been attempted. Part of the reasoning for this is that MRI is a very expensive imaging tool, not only for running the machine but to have a professional reading of results. But be aware it is not just cost that limits the immediate jump to performing MRI. There is also the fact that for as expensive of imaging as MRI is, evidence and experience have shown that MRI can be inconclusive to diagnosis or even that after round of physical therapy/ proper expanse of time many issues can clear up on their own. When injuries are acute, they can be particularly painful but given time and rest the body works to heal itself and it may be found that symptoms greatly reduce or completely disappear. Cases can occur of MRI showing no abnormal results or even only mild inflammation despite symptoms and pain, but there is also the fact that an MRI can reveal conditions in the body that can be normal or asymptomatic such as a bulging disc or normal wear and tear of aging such as arthritis. Despite not having a clear diagnosis such as imaging, physical therapy can still prove beneficial. A physical therapist upon initial evaluation will take measurements for range of motion and manual muscle testing to determine if someone has limitations that could contribute or be due to certain conditions. Additionally, Physical Therapists are trained to perform special tests that can be indicative of certain conditions. At your initial evaluation, your Physical Therapist will make a diagnosis and a plan of care in order to regain strength, mobility, and function. Why would I do physical therapy following surgery? Despite the great effort of surgeons and the treatments, they perform there is not always a guarantee of full recovery or complete dismissal of pain with surgery alone. Physical therapy can work to help the surgical recipient work through the pain and regain function. Additionally, many times surgery is performed with the goal that therapy will help maintain and progress strength/range of motion/function during the recovery phase. Some surgeries require precautions such as a brace or sling, limited active motion, or completely prohibited use of a limb. For example, following a rotator cuff repair a sling is worn roughly 6 weeks and active motion or use of the arm is not allowed, this is because the repair needs time to solidify. While the active range and use of the arm shouldn’t be performed, your physical therapist will move the arm for you in order to maintain the flexibility in the shoulder. This process is necessary so that the arm is not “locked up” upon the phase of recovery where active motion is being re-established. Another example of a common surgery that typically requires physical therapy following is a total knee replacement. Following a total knee replacement, there is quite a bit of swelling, pain, stiffness, and weakness; A physical therapist will help maintain and progress the range of motion of the knee, begin gentle strengthening, and over the course of recovery help the patient regain strength/function of the lower extremity. Working with a physical therapist can also include adjusting back to walking without an assistive device or lesser assistive devices such as from walker to cane or from a cane to unassisted walking. Conclusion: Whether or not you choose to have surgery is between you and your physician. The best you can do for yourself is research your condition, keep an open mind to your Doctor’s recommendations, and give your best effort to any and all treatments suggested. If you do Physical therapy expect that you will not have instantaneous results. With Physical therapy, a home exercise program will be assigned for you to perform daily and it is important that this is performed. The home exercise program is the foundation of your recovery and if all you do is the activities within the clinic for therapy, you are ultimately limiting your own success. If you end up requiring surgery, research the surgery and learn about the process it will take to recover. Pain and the sometimes-arduous journey of recovery can make you feel isolated, but you must remember that you are not alone and that many people have gone through and will go through these same issues. After surgery patients will typically receive an order for outpatient therapy from their surgeon or doctor. This order can be started any time from a couple of days to a few weeks after the surgery is completed. At this time, patients may have noticed some improvements in their ability to move the body part that was operated on and a reduction in pain associated with movement; however, their body is still healing and as the body heals it creates scar tissue to close the wound. This is a regular part of the healing process but if we allow the body to produce this tissue without appropriate treatment, it can lead to decreased range of motion, reduced functionality, and in some cases can lead to manipulation under anesthesia, or in rare, more severe cases, revision surgery. By attending physical therapy and receiving ASTYM along with other treatments, these patients are taking the necessary steps to increase the chances of a successful surgery, a better quality of life, and reducing the possibility of further, unnecessary, hospital visits. What is ASTYM and how does it work? ASTYM is a soft tissue technique that works to improve the healing process for muscles, tendons, and skin that have been impaired by trauma, injury, or surgery. ASTYM enhances the healing process through micro-trauma. This micro-trauma is stimulated by the tools that the therapist uses to rub along the tissue and is similar to the micro-trauma that occurs in muscles during resistance or strength training. This trauma increases the amount of blood that goes to the treated area, bringing with it all the nutrients required to start rebuilding healthy tissue in the affected area. Next, the physical therapist will provide the patient with specific stretches and strengthening exercises designed to control the direction and length of the new tissue, to allow for a functional range of motion and strength. What does the research say? There are several quality research articles that all come to the same conclusion. Patients that have had surgery, or any soft tissue injury that received ASTYM therapy have better outcomes and function at the end of their physical rehabilitation than those that received a standard of care protocol or exercise alone. In a study conducted by Sevier and Stegink-Jansen, they found that 83.6% of patients treated with ASTYM for elbow tendinopathy, met resolution criteria for their condition. Meanwhile, only 40.9% of patients treated with exercise alone met resolution criteria.1 In a study looking at the benefits of ASTYM in combination with other modalities in patients after their first total knee replacement, conducted by McGinn, Chughtai, Bhave, et al. they found that only 2% of the patients treated with ASTYM required manipulation under anesthesia as compared to the standard of care group, in which, 13% of patients received manipulation under anesthesia2. In a second study that looked into the benefits of ASTYM in patients that had received a manipulation under anesthesia after a total knee replacement found that there was a “significantly higher proportion of patients who achieved an optimal ROM in the IMMPT cohort compared to those who were in the standard group”.3 This study also found that the patients who received ASTYM therapy had an average knee flexion range of motion of 116 degrees compared to those that didn’t, who had an average of 106 degrees3. That correlates to better function of the knee when it comes to activities such as stepping over obstacles that might cause the patient to fall or even just walking up a flight of stairs without catching their foot on a step. To further illustrate the benefits of ASTYM therapy, a study conducted by Davies, Brockopp, and Moe, studied the benefits of this treatment in patients following a mastectomy. They found that the women in this study were able to improve their ability to move their arm out in front of them by 17 degrees, and out to their side by 19 degrees4. This essentially means that these women went from not being able to reach into their cabinets for dishes and needing to ask for help, to being able to be more independent and able to perform everyday overhead tasks better. This study also found that these women had significantly improved their outcome measure scores, and more importantly their clothing tolerance.4 Meaning that they were able to wear their normal clothes without experiencing pain or irritating sensations. The final study that will discuss was conducted by Kivlan, Carcia, Clemente, Phelps, and Martin. They studied the effects of ASTYM therapy on muscle strength and found that patients who received ASTYM therapy increased their maximum force output of the leg immediately following treatment by an average of 15% compared to those that received the control or placebo treatments, which demonstrated a 1% and 6% decrease5. In conclusion, patients who have had a surgery or trauma benefit from seeing a physical therapist that is skilled in ASTYM therapy. By doing so, these patients significantly reduce their risk of needing more treatments after surgery and increase their chances of better overall function of the injured body part, in a manner that allows for quicker strength return and greater range of motion in the long run. Does it take all you have to make it to the gym to workout because your joints hurt? Did you know that there is another type of exercise that decreases the amount of stress on your joints, but can still give you the same strengthening and cardiovascular endurance benefits that traditional gym workouts can give you? There are many benefits to aquatic therapy, including decreasing swelling, increasing circulation and dynamic strengthening. The water buoyancy and decreased weight that is put on joints that are forced to otherwise bear weight on land, are able to move freely and can be put under greater stress sooner for earlier mobilization (1). Osteoarthritis is the most common type of arthritis. According to one study, the lifetime prevalence of knee OA is 44.7% and the lifetime prevalence of hip OA is close behind, with 25.3%2. Common symptoms of OA include sore or stiff joints that go away after movement or activity, limited range of motion, clicking or cracking sound when the joint bends, mild swelling around the joint and pain that is worse first thing in the morning or towards the end of the day. You may notice that the pain is in multiple joints or in joints that you have had injuries to. For example, both knees or both hips or in joints that you have injuries or were operated on. The pain typically will get progressively worse as you age, but there are things that you can do to stay active, such as aquatic therapy! In one study conducted by Tapani et. al that looked at the effects of static and dynamic strengthening exercises on knee flexors and extensors in both water and on land, the researchers found that aquatic therapy was a good alternative for strengthening for those patients that were otherwise limited in their abilities to strengthen on land (3). They found that patients had increased muscle activity and gained lean muscle mass in both their quadriceps and their hamstring muscles. Another study conducted by Uthman et. al performed a systematic review of various treatment options for osteoarthritis, many in combination with each other. They also looked at the two most common types of osteoarthritis to compare what treatments would be the most beneficial for hip and knee OA. They found that strengthening/flexibility in conjunction with aquatic therapy and aerobic exercise was the best way to treat OA, all of which can be gained while in the pool. Aquatic therapy, while it is a great treatment option for people with osteoarthritis, it is also a fun way to stay in shape and help to decrease your joint pain. Most YMCAs have water aerobic classes that you can participate in or you can find resources on the internet as well. The Arthritis Foundation has a section dedicated to aquatics and pool therapy (4). There are also examples of exercises online to get you started in the pool. So what are you waiting for? Grab your suit and give it a try! 1. The gluteus maximus is the largest muscle in the body especially in terms of density and mass, the glute muscles also have a large job including maintaining posture of the trunk and supporting the weight of the body. There are three sets of gluteal muscles largest being the gluteus maximus, the glute medius which controls sideways movements and stability of the hip, and glute minimus the smallest which mainly functions to avoid “falling in” of the leg while in use. Typically, the most prominent cause of weakness in these muscles is lack of activity or sedentary lifestyle. As technology advances and more people are confined to desk jobs in which most of the day is spent in a sitting position, the glutes atrophy and the anterior hips become accustomed to a shortened position. The issue that this creates is that the anterior hip, or more accurately described as hip flexors, become tight and can alter posture of the pelvis, hips, and lumbar spine. As posture is altered the glute muscles find themselves even less relied upon due to improper body mechanics during activity. Atrophy can also be seen in the glutes following an injury or post-operatively from a surgery to hip/knee/or back that causes the person to be unable to perform activities that work these muscles, for example being in a back brace for 2 months or non-weight bearing on crutches for 6 weeks.   How do I strengthen my glutes? +Squats- either full standing, (or up and down from chair), (or mini squats) +Clamshells +Bridges, (single leg bridges), (bridges with upper back on swiss ball), (bridges with feet on swiss ball), (bridges with hamstring curl on swiss ball) +Hip extension in standing (or while on hands and knees) (or while lying on stomach) (or on stomach over the swiss ball) +Sideways leg lifts while lying on side (or standing) +Stairs +Donkey kicks +Fire hydrants +Glute sets +Resisted sidestepping and monster walks If you have issues with knee pain, back pain, or bad posture it may be a good idea to begin some gentle strengthening such as glute sets where you squeeze and hold your bottom. Bridges within pain-free range where you tighten the bottom and try to lift while lying on the bed or floor with your knees bent. Clams in which you lay on your side with knees bent and open and close the knees while keeping the feet together. If any of these activities are very difficult it is likely an indication that your glutes are weak or inactive. If that is the case, continue gentle glute exercises and progress exercises as you get stronger. Always be cautious with new exercises as going overboard with reps and sets or weight can cause delayed onset muscle soreness which results in sometimes severe soreness of the muscles 24-48 hours following exercises. If possible, try to find a source of instruction for new exercises such as video, handout or instruction from a professional as having good form will be most effective and help avoid un-necessary soreness. Our joints need synovial fluid (lubrication) to stay healthy and move well. The best way to circulate synovial fluid is through mobility. There are several low-impact exercises and activities that are good for people with osteoarthritis. Walking, biking and swimming are just a few examples. Activities should be modified for good days and bad days. When you are having less pain, it would be a good idea to do some higher-level exercises and even incorporate some strengthening exercises. On days that you have more pain, it would be good to take these days easy and incorporate more stretching exercises. Whether you are having a good day or a bad day, it is always good to warm up with some light exercises as well as cool down with some light exercise. Whenever you start a new exercise program, some pain, stiffness, and swelling are normal. Don’t let this discourage you! It may take several weeks (up to eight) before your body gets accustomed to the new activity level. Making sure you have good, supportive footwear will help with this. It is a great coincidence that World PT Day occurs during Pain Awareness Month. - [Everything you need to know about overcoming plantar fasciitis.](https://aptclinics.com/blog/everything-you-need-to-know-about-overcoming-plantar-fasciitis/): The weather is finally starting to cool down and you want to enjoy the crisp air, but have been experiencing a pain in your heel and bottom of your foot. You’re not alone. This symptom is often caused by plantar fasciitis. The fascia is a thick band of tissue that attaches from your heel to your toes. This area can become overly stressed or stretched causing micro-tears, pain, and inflammation. Bone spurs can also form and be present at the heel area which can further tissue aggravation. Possible causes of Plantar Fasciitis are: Wearing worn out shoes, no shoes, or shoes with very flat thin hard sole during standing for long periods, walking, or other vigorous weight-bearing activities. Having flat feet or a very high arch can predispose towards this condition. Abnormal gait or foot positioning during activities such as walking or running. Tightness at Achilles tendon. Symptoms: Plantar fasciitis can include pain at the heel or bottom of the foot, especially when walking long distances or the "first step" of the morning/ standing after sitting for a long period. Plantar fasciitis can progress to the point where even short periods of walking are painful. How can we treat this with Physical therapy? Exercises for strengthening the arch and smaller muscles surrounding the toes. Ankle strengthening to avoid abnormal mechanics during gait, standing or running. Stretches for calf and bottom of the foot to decrease tension to the Achilles area, plantar fasciae. Some modalities for pain relief and or increasing blood flow such as Ultrasound or electrical stimulation. Manual therapy including manual stretching of ankle/foot or techniques for reducing tissue texture or knots, ASTYM therapy. Kinesiology tape may be applied in some cases to help decompress the plantar fascia and provide support to the arch/ inhibit tightness in calf musculature that increases tension on Achilles tendon. What can I do for myself? In some cases, arch supports for shoes may be necessary, also avoiding wearing worn out or hard flat shoes with little to no arch support. Find shoes that provide good support for the arch and ankle, promoting good ankle and foot alignment during stance, ambulation and running. With education from a physical therapist, you can learn to tape the foot and ankle so that you can walk longer distances or do an activity without pain. Follow your home exercise program assigned by your physical therapists, and stretch daily. Rolling pin or a frozen water bottle along the bottom of foot and heel. Wearing night splint to hold foot and ankle in a neutral position rather than toe pointed down. These shows are designed to provoke discussion and disagreement, but usually, this disagreement occurs with opinions, not facts. In this episode, the hosts Max Kellerman and Stephen A. Smith discussed the status of the Steeler’s franchise quarterback. “He’s had concussions before,” Kellerman said. “You know what the rehab is for concussions? Nothing! There’s no rehab for concussions!” Smith’s response was equally as concerning. “There’s concussion protocol, and that’s it.” Kellerman further perpetuates the falsehood by stating, “What are you going to do about a concussion? Yeah, wait, go through the protocol. What does that mean? Oh, don’t do anything, hope your brain heals.” The reality is that physical therapists can undoubtedly treat many of the symptoms experienced with a concussion. As someone who has sustained a concussion, I understand many of the frustrations that come along with the resultant symptoms. These issues can be, and no two concussions are the same. Headaches, dizziness (including vertigo), difficulty concentrating, difficulty with vision, ringing in the ears, and nausea can all be attributed to concussions. The question is, how do we treat this? The brain is a complex organ that controls almost every aspect of our actions. When it is injured, many other systems in the body can be affected. A concussion is a brain injury that usually occurs as the result of a strong force to the head or neck area. This force can cause a “coup-contrecoup” action where the brain impacts against the skull. As physical therapists, there are three primary systems we treat when concussions don’t heal with just rest. These are the oculomotor system, vestibular system, and cervical spine. These three systems also happen to be more connected than one may think. When treating oculomotor deficits, a physical therapist will first assess the quality of motion when you move your eyes. They will also test the patient’s ability to move their head when with a fixed gaze. These deficiencies can lead to may symptoms including double vision and headaches as your eyes strain to keep focus. A physical therapist can provide exercises designed to address the patient’s specific dysfunction to rehabilitate the system to full function. The vestibular system can be quite complex with many of its functions grounded in the tiny inner ear. Such a small organ can have a profound effect on the function of the body. With concussions, it is common to experience dizziness, imbalance, or even vertigo. The cause of these conditions can be numerous. A physical therapist who is trained in the vestibular system can diagnose and treat many concussion-related causes of this dysfunction. When returning to sport or work, it is vital that this system is rehabilitated completely. We see in the research that concussions can potentially increase the risk of a lower extremity injury. Making sure all the systems of the body are appropriately rehabilitated is a necessity before returning to work or sport. Neck pain and stiffness are quite possibly the most common symptom in the post-concussion spectrum. Many of the headaches experienced after a concussion tend to originate in the neck. The cervical spine is full of sensors that give signals to the brain that let the brain know where the body is in space. When the neck is stiff and painful, these signals can be inaccurate. Add the fact that the brain itself is healing and injured, and the results can be quite debilitating. The goal of the physical therapist is to improve the function of the neck and rehabilitate it to full health. We can use many treatments including dry needling, soft tissue mobilizations, manipulation/mobilization, and exercises to promote the healing progress. As with the vestibular system, rehabilitating the neck to full health is vital to avoid further injury. We see in the research that increases in neck strength can potentially decrease the risk of concussions. When the neck is stronger and can stabilize the head, there is less of a force absorbed by the brain. We are learning more about concussions at a rapid pace. New research is published monthly, and as we learn more, we are better able to treat concussions. Spinal stenosis occurs most often in the low back and neck when degenerative changes happen at the discs and areas of the bones that make up the spine. This can additionally cause bone spurs, put pressure on the spinal cord and nerves that branch off this area due to narrowing of the spinal canal. Symptoms of stenosis can include numbness or tingling sensations down the legs or arms depending on the level of the spine affected such as cervical or lumbar areas, also pain or difficulty with walking or exercising, and weakness. When spinal stenosis occurs, it can also be caused by spinal injury or bulging/ herniated discs. Whatever the cause, the mobility of the spine is reduced and therefore the natural curve of the spine is affected. Stenosis tends to be a condition that gradually worsens with time and can reduce activity due to the progressive nature of symptoms. Because of this, many people with advancing stenosis find themselves unable to do normal daily activities and requiring more rests due to weakness and or pain. When muscle atrophy occurs due to this lack of activity and reduction in capabilities it can worsen the symptoms of stenosis as the muscles are strong supporters of bony structures and healthy joint motion. How will physical therapy help? First, with physical therapy, we assign several stretches to assist with tight muscles surrounding the spine and improving the mobility of the spine. Most, if not all exercises, will include some time of forward bending to re-establish motion in the spine as well as to help "open" the spinal canal and take pressure off the spinal cord and nerves. Additionally, there are activities that will glide the nerve along its pathway, and by putting tension on and taking tension off the nerve, we can help establish mobility and reduce restrictions that can help decrease neural tension that causes numbness/tingling. Okay, but what about cervical stenosis? Cervical stenosis can be a little bit trickier depending on the severity of symptoms. Typically, we will start stretches for the upper trap and levator, scalene musculature along the neck. Additionally, we will look at postural muscles and posture itself. A forward head and forward slumped and rounded shoulders can contribute to pain, weakness, and numbness or tingling in the upper extremities. Your Physical therapist will assign exercises to stretch the chest and strengthening for the mid and upper back to help re-establish a good balance between the front and back of the torso and to encourage good posture. They may also assign nerve glides for the upper extremities if a radicular pain is traveling down the arms or hands. What can my Doctor do to help? Your Doctor can order imaging and provided a diagnosis of your condition, they can recommend or prescribe physical therapy or in moderate to severe cases may direct towards orthopedic surgery or specialist in Neurology. In some cases, prescription medications can be assigned for assisting with tight or cramping muscles, numbness or tingling/ nerve pain, pain in the neck or back. In some cases, steroid injections have been used although increasing evidence of weakened bones and connective tissues with repeated injections has more Doctors concerned about this practice. Should I wear a back brace? Back braces can be great for protecting the back during particularly heavy work or lifting for specific short-term scenarios. E.g. lifting a couch, taking heavy items up the stairs. The issue that occurs with the use of back braces is that long-term use and wearing of these devices can weaken core musculature and cause atrophy that can further increase pain when the brace is not being worn. For example, say you wore a back brace for 8 hours a day, for that time none of the core muscles must work to support posture or engage to do daily activities. So, over a period of one month wearing the brace 8 hours a day, you are propagating 240 hours of muscles atrophying and core muscles becoming "lazy" at their job even though you have been working away at your own activities. At the end of the day, what can you do? Stretch, strengthen, and listen to your physical therapist. Keep doing the hard work and you will eventually prevail. In order to reduce your chance of ending up as a wounded weekend warrior, though, stretching is key to preventing injury. Dynamic stretches, stretching while moving, are great for before you exercise. Make sure to address the major muscle groups you will be using during your activity (ex. Hamstring, glutes, calves, hip flexors and quad for running). After your activity static stretches, stretches you hold for 20-30 seconds, are good for helping to build your flexibility in order to further reduce your risk of injury and pain. Most of the time, when a patient reports no progress, it turns out they struggled to commit to the process throughout their treatment. Dry needling, which is a different procedure from acupuncture, has been shown to be a safe method for addressing trigger points. A myofascial trigger point is characterized by a taut band of muscle that is tender with palpation. These trigger points can cause localized and referred pain, restrictions in mobility, and weakness within an affected muscle. There is decreased blood flow and oxygen saturation, as well as increased acidity and concentration of inflammatory substances, present within a trigger point compared to normal muscle tissue. It has been shown that several of these inflammatory substances decrease 5-10 minutes following dry needling treatment. Stimulation is applied to treatment points relating to different systems involved with nerve impingement or ANS (Autonomic Nervous System) up regulation. Patients re-assessments performed after treating each system can determine the extend of therapeutic involvement. Using MPS, the assessment and the treatments of the root causes of soft tissue pain can be completed with a high degree of accuracy within minutes. MPS Therapy not only provides pain relief, but it improves ROM and releases fascia in a fraction of the time it takes manual therapies or other modalities. The nerve carries both sensory information (what we feel) and movement information (how we move). Therefore, once the specific nerve root level is determined, the therapist will treat the sensory and movement portions of all the muscles along that nerve root.
